Tutorials: Each person may select one morning tutorial and/or one afternoon tutorial from the following list. No hopping between tutorials is allowed.
You may submit your registration on-line (recommended method) or by fax, no later than Tuesday, May 15, 2007. After this date, only on-site registration is possible. Registration by e-mail or phone will NOT be accepted. The registration options may be exercised at the end of this page. Please make sure to carefully read all the following information, first. Your registration payment will be processed through a secure web-based credit card payment system (SSL encrypted data transfer). ADDITIONAL REGISTRATION APPLICATION Those who would like to only attend tutorials without registering for the conference or those already registered, but would like to add additional items (i.e. register for tutorials, or get the tutorials book, ot get additional banquet tickets, etc.), please use the ADDITIONAL Registration Form. The additional registration application cannot be submitted on-line. Please fax (+1 312 604 7376) the ADDITIONAL Registration Form. Confirmation will be sent via e-mail, after proof of payment has been received. Please remember to print and bring this confirmation letter with you to the registration desk. All payments must be made in US Dollars. Registration fees can be remitted by credit card (Visa, MasterCard, Discover, AMEX are acceptable) or bank transfer. Cash payments will only be accepted on site. Credit card payment will also be available on site. No other type of payment will be accepted. • Pay on-line using your credit card. Your credit card statement will show the charge as: Registration Assistant. You will also receive the official confirmation of registration via e-mail from the ISCAS 2007 registration desk. • Pay by faxing the PDF registration form with your credit card details. Then, the ISCAS 2007 registration desk will charge your credit card with the respective amount and send you the official confirmation of registration via e-mail. • Pay by bank transfer. Whether completing your registration on-line (using the Bill Me (wire transfer) option) or by faxed form, your registration will not be considered complete until we have received your wire transfer to the conference bank account. For quick and accurate processing of your registration, it is requested that you fax a copy of your bank transfer receipt to +1 312 604 7376. Please remember to also bring this copy to the registration desk. After proof of payment has been confirmed by our bank, you will receive the official confirmation of registration via e-mail from the ISCAS 2007 registration desk. Please note that any additional transfer or service charges imposed by the bank must be paid by the participant. DO NOT allow your bank to deduct it from the registration fee.
CONFIRMATION OF REGISTRATION Upon receiving your application and fee, the registration desk will e-mail you confirmation, including your registration number. Please remember to print and bring this confirmation with you to the registration desk. CANCELLATION POLICY Cancellations must be made in writing, either by fax (+1 312 604 7376) or e-mail (iscas-reg@amg-corp.com). The amount of refund will depend on the date of cancellation, as follows: Refund Policy
A total refund of all prepaid fees will be made, excluding an administration charge of $125, provided that advance written notification is submitted by April 15, 2007. No refunds will be made for cancellations received after April 15, 2007. All refunds will be issued within 30 days after the end of the conference. Delegates requiring a letter of invitation in order to attend the Conference may write to the Conference Secretariat specifying the necessary details. Please note that this procedure aims to assist delegates who need to obtain a visa or permission to attend the Conference. It is not an official invitation covering fees and other expenses and does not imply any financial support from the Conference. Attendees who need visas should apply for it to the respective consulate/embassy no later than 3 months prior to the conference. For visa information, please visit: http://travel.state.gov/visa/index.html Personal Insurance The organizers cannot be held responsible for accidents to conference participants or accompanying persons, for damage or loss of their personal property, or for cancellation expenses, regardless of cause. Participants should therefore, make their own insurance arrangements. Please consult with your local insurance sources regarding coverage.
